Covid-19 Vaccinations in school – Tuesday 12th October

The Covid-19 Vaccination Team from shropcom.immunisationteam@nhs.net will be in school on Tuesday 12th October. All students aged 12-15 years and with full parental consent can be vaccinated. All relevant information is here:

Covid-19-Vaccine-Info-Letter-for-12th-October

and the necessary consent form can be found here:

Schools-COVID-19-Consent-form-12-15-year-olds-v2-002

The consent form must be fully completed, signed and returned to school by Friday 8th October in order that the vaccination team can be advised on numbers. The NHS letter suggests that students who are 16 and over do not need consent and can still have their vaccinations in school on the day. This will not be possible due to the high volume of 12-15 year olds and are therefore encouraging them to use their local walk-in centres. Information on these can be found here: https://stwics.org.uk/our-priorities/covid-19-vaccination-programme/walk-in-clinic-times

Any queries or concerns regarding these vaccinations must be directed to shropcom.immunisationteam@nhs.net or by telephoning 01743 730028. Staff at Holy Trinity Academy cannot give advice or information on this matter.
